The accident resulted in 8 crewmembers sustaining fatal injuries, and 17 others suffering injuries ranging from serious to minor (National Transportation Safety Board [NTSB], 2007). The prima facie causes of the accident according to the report were: 舦 The lack of adherence to water chemistry composition limits and procedures by both the water chemistry subcontractors and shipowners during wet lay-up periods, leading to pitting from oxygen corrosion.
In other words, accident causation is not only about people (as individuals or teams) or the technology they work with alone. Significantly, it is also about places (the organizational systems and associated cultures) in which they work. 舖Places舗 here refers to, not only physical locations, but also to social settings of interaction. The multiple-factor causality of accidents and deviations from quality is well-documented (Hollnagel, 2004; Reason, 1990, p. 197; Wagenaar and Groeneweg, 1987). While acknowledging this, the current study focuses on those factors that impinge on the inability of the operators to act as a team, the lack of 舖confidence舗 of subordinates in contributing safety-critical information to other organizational members, and the non-existence of a learning culture where this is required.
As part of this investigation, the investigating officer visited a sister vessel, the Bow Transporter, in Singapore. During that visit many of the same attitudes were observed. The Filipinos were only permitted to speak to the investigating officer and Singapore officials in the presence of the senior officers, leading to obvious nervousness. Nevertheless, several crewmembers made statements confirming the same cultural divide existed aboard the Bow Transporter. The chief officer reported that he planned all of the cargo and tank cleaning operations, and also remained awake throughout all cargo operations.
